Submit Your Club’s Annual Meeting Delegate Roster and Dinner – Gala RSVP
DEADLINE: May 5, 2019 until 11:59 pm, no exceptions.
The number of delegates is based on your club’s membership as entered in the membership portal as of March 1 of this current year. Delegates must be 18 years and older as of March 1 of this year.
Official Club Membership List
The list of your club’s qualified members and maximum allotted number delegates was emailed to your club president. That person should be submitting this form.
The delegate roster (below) indicates the delegates chosen by your club to vote at the Annual Meeting.
Please enter the delegate names and membership numbers of those representing your club in the delegate roster form. Also include the names and membership numbers of alternates that will replace a delegate(s) who is unable to attend the Annual Meeting.
Only those named as alternates may replace a delegate. No other substitutions are permitted.
All members are welcome to enter into discussions, but only persons named as delegates or named alternate delegates (when replacing a delegate) may make motions, vote and otherwise participate in the business meeting.

The Dinner and Gala are open to all club members. RSVP on the form below. Send a check along with your payment of $35 for each delegate and guest who wishes to have lunch.
